		<description>It is with great shock and dismay that we learn of the death of
Michael &#039;Lorro&quot; Molokwane who was a Miner at Impala Platinum Mines
,at the notorious 14th Shaft located in Mogono Section of Luka within
the Royal Bafokeng Nation.

I am writing to the North West fm in particular to the View Point
Programme as i believe that your role to curb this scourge of mine
Deaths and disguised accidents should come to a halt.

On behalf of Mogono Community Development Trust,and as custodians of
our residents we learned on the 25th of February 2011 of the passing
away of Michael &#039;Lorro&#039; Moklokwane nee Rangaka.ID NO 7308155779085 who
was working as a shift supervisor at the notorious 14th Shaft of
Impala Platinum Mines.

The flimsy report on the cause of his death according to the mine, is
an accident underground wherein the deceased wash smashed by a
so called sgubu (underground loader) under very dubious circumstances.

I am saying so because of the following.
1.	 The deceased refused to work any substandard work in his career and
his colleagues  can attest to this effect. It was the deceased who
Inspected the scene on the day when the rock fell and killed
9 miners at the same shaft last year and when he refused to allow his
shift to work in the unsafe area, the deceased was substituted by another
Supervisor who took the miners to an extremely dangerous site and they
Were killed by a rock fall.
	2.  Subsequent to this incident the deceased formed a miner&#039;s forum to
avert such incidents in future and correct other atrocities happening
at the shaft.
This did not auger well with his shift boss, mine captain and the
bottom shaft managers who happens to be all white with a history of racism.
  	3.  Another death occurred and the deceased was very vocal of the
substandard work happening at the shaft and he became the enemy of the
mine.

Before his untimely death ,the deceased had a case against the shaft
manager wherein the mine manager insulted Bafokeng women working at
the mine.
4.  A day before the “accident” the mine captain and the shift boss
wanted to talk to the deceased who then refused to talk to them .On the
day of the &#039;accident &#039;they also tried to convince him to drop the
charges against the shaft manager and he refused to talk to them. They
proceeded to have a 20minutes meeting with the trackless machinery “sgubu” operator who
within an hour after the meeting drove the machinery off track and crushed into, and killing the deceased underground. The operator ignored the light signals from the deceased’s head lamp and the screams from the deceased loud voice warning the operator of the impending impact. The
“sgubu” operator thought he was with the deceased only ,to his dismay
there was two witnesses who saw the whole incident and when he realized that
he ran away.
5.  The first people to be at the scene of the &#039;accident&#039; are the very
same people who had the meeting with the “sgubu” operator before the
accident to come and confirm their mandate.

This is extremely brutal to the son of the soil.
our plea is .

As a community of Mogono we want a commission of enquiry on the
So called accident.
The sgubu operator to be arrested together with the mine captain and
the shift boss with immediate effect pending the outcomes of the
investigations.</description>

I&#039;ve heard about a horrible accident on the N17 on Sunday evening - 20 Feb - round about 20H30 - 21H00 where a whole family of Secunda was wiped out. It happened on the new section that was recently opened and where they are still constructiong the toll gate - close to Leslie. Do you know anything about this accident? Apparently it was 3 cars involved but I can&#039;t find any detail on any webpage nor any newspapers. Thank you</description>

I have the unenviable task of convincing managers at companies within our group that fatal accidents could happen to their employees and the responsibility they have in ensuring that this does not happen.  Having accounts of actual accidents both in the industrial sector and farming sector would go a long way in convincing them that it could indeed happen to them if they don&#039;t act.  Can you recommend any sites that refer to such accidents or have statistics?  Also, could you perhaps include the year in the date, as the latest information is always more motivational in these cases!

I am also a photographer and have had meetings with the Manager of Media for the EMS and he has given me permission to take photos on calls such as car accidents and fires. I am always on scens taking photos and afterwards hand them over to the investigating officers but then left with nowhere to go with my photos. I was at the old age home that burnt on Sunday evening and took some great photos with Chris Botha from Netcare who&#039;s photo is on your website. Is it possible that I can send my photos through to you whenever there is a big call, to put onto your website, or how does it work.

I take photos of the call only, I would never take pictures of bodies or any photos that will include faces, car reg numbers or anything like that. I want to use my photos to warn people of the dangers out there.

Please could you let me know what I can do to get these on a website to help people out there.
